FRIDAY, giving 48 hours notice please, by:
- Using our Online form -

Write the name and the dose of the medication onto
the reorder slip, and the doctor will assess your

the surgery and don't have a computer?
Post your reorder slip to us, and if you indicate,
then your chemist can collect the prescription from
the surgery for you.
